Job Description

We are currently seeking a dedicated Nurse Staff member to join our team at Providence Community Health. As a Staff RN, you will play a crucial role in assessing and addressing the health care needs of individuals and families across all age groups. This position requires a commitment to providing high-quality care to a diverse patient population in a community health setting.

Education/Qualifications

• RN from an accredited school of nursing or college preferred.
• Current R.N. Rhode Island state license required.
• One year nursing experience in hospital, public health, or community health preferred.
• Experience in managed care patient services.

Essential Duties & Responsibilities

• Assess the health care needs of individuals and families.
• Develop nursing care plans based on age-appropriate and cultural considerations.
• Case manage families to achieve health care goals.
• Provide specialized nursing services and treatments.
• Manage clinic flow and coordinate patient care.
• Document all care and communicate effectively with providers.
• Maintain knowledge of healthcare programs and resources.
