We are looking for a dedicated Nurse Practitioner or Physician Assistant to manage a patient panel at one of our employer on-site clinics in New York City. This role involves treating patients in-office, conducting tele-health visits, and collaborating with in-office and virtual clinical teams.

Responsibilities

• Manage a diverse patient panel with acute, chronic, and well visit needs.
• Collaborate with in-office and virtual clinical teams for comprehensive patient care.
• Utilize specific clinical training to perform in-office procedures.

Requirements

• Completed accredited NP or PA program with national certification.
• 2 years of experience in outpatient primary care or 1 year in outpatient primary care with additional training.
• State license in New York.
• Passion for human-centered primary care and evidence-based practice.

Salary: $127,000 to $135,000 per year based on a full-time schedule with potential for additional incentives based on qualifications and performance.

This is a full-time role based in New York, NY, in partnership with Mount Sinai.
