Nurse-LPN MDS Coordinator

Job Description

We are looking for a dedicated Nurse-LPN MDS Coordinator to join our team at Genesis Healthcare. As the LPN MDS Coordinator, you will be responsible for participating in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, following all guidelines for completion, encoding, and electronic transmission of the MDS.
• Assist in developing individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Current LPN license in good standing.
• Prior experience in a healthcare setting.
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficiency in Windows applications; experience with AccuMed is a plus.
• Previous MDS Coordinator experience preferred.
• Certification in BLS/CPR required.
