Job Description

We are looking for a dedicated Nurse Case Manager (RN) to join our team at Sage Health in the Baltimore Market. As a Nurse Case Manager, you will be responsible for implementing and reviewing healthcare plans for patients with geriatric, recovering from serious injuries, or dealing with chronic illnesses. You will collaborate with doctors and medical professionals to provide comprehensive care, advocate for patients, coordinate care, and offer healthcare services and education.

Responsibilities

  • Ensure patient information security and confidentiality.
  • Create and manage care plans for patients with chronic or serious conditions.
  • Advocate for personalized treatment options for patients.
  • Communicate with patients and families about health conditions.
  • Coordinate post-discharge appointments and follow-ups.
  • Provide guidance and education to patients for complex medical decisions.
  • Liaise with insurance providers to promote quality and cost-effective care.
  • Maintain electronic health records and update medical treatments.

Requirements

  • B.S. Degree in Nursing.
  • Active Registered Nurse license in the state of employment.
  • Basic Life Support certification.
  • 2-3 years of experience in case management in healthcare settings.
  • Strong clinical assessment skills and experience with chronic disease management.
  • Excellent interpersonal and communication skills.
  • Ability to work in a fast-paced environment and manage priorities.
  • Resourcefulness in problem-solving and attention to detail.

Preferred Qualifications

  • Case Management certification (ACM or CCM).
  • Experience in managed care (e.g., Medicare HMO, D-SNP).
  • Motivational interviewing skills.
  • Bilingual in Spanish and English.

Physical Requirements

Visual acuity, hearing, standing, walking, and lifting are essential components of the role.
