Job Description

Genesis Health is seeking a dedicated Minimal Data Set Coordinator - LPN to join our team. As the LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS). Your contributions ensure proper patient care and nursing center reimbursement.

Position Highlights

• Direct and oversee the interdisciplinary RAI process, ensuring compliance with guidelines for MDS completion, encoding, and electronic transmission.
• Develop individualized resident Care Plans, addressing resident needs and expected outcomes in line with Federal and State regulations.
• Train new interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license in good standing.
• Prior experience in hospital, long-term care, or healthcare facility.
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficiency in MDS and Care Plan completion.
• Knowledge of RAI/PPS processes.
• Computer skills including Windows applications; AccuMed experience preferred.
• Current BLS/CPR certificate.
