Join Genesis HealthCare as a Minimal Data Set Coordinator - Licensed Practical Nurse. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS). You will ensure appropriate patient care and nursing center reimbursement through accurate assessments.

Responsibilities

• Direct the interdisciplinary RAI process, following completion guidelines.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train the team on clinical content and electronic completion of MDS items.

Requirements

• Current LPN license in good standing.
• Prior experience in hospital or long-term care facilities.
• Proficiency in general nursing practices and medical regulations.
• Computer skills, including Windows applications.
• AccuMed experience preferred.

SHARENLMD
