Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a licensed Practical Nurse to join Genesis Health as a Minimal Data Set Coordinator. As part of our healthcare team, the LPN MDS Coordinator will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) to ensure quality patient care and proper center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in compliance with guidelines for MDS completion and transmission.
• Assist in developing personalized resident Care Plans in adherence to Federal and State requirements.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in healthcare facilities.
• Familiarity with nursing practices and regulations in long-term care.
• Proficiency in computer applications, Windows, and AccuMed experience preferred.
• Past experience as an MDS Coordinator.
• Knowledge in completing MDSs and Care Plans.
• Competent in RAI/PPS processes.
• Current BLS/CPR certification.
