Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a detail-oriented Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our dedicated team. As the LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Direct the RAI process and ensure accurate completion and transmission of the MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• Licensed Practical Nurse in good standing with state licensure.
• Prior experience in hospital or long-term care facilities.
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficient in computer applications, preferably with AccuMed experience.
• Previous MDS Coordinator experience preferred.
• Current BLS/CPR certification required.
