Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a talented Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our healthcare team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Responsibilities

• Direct the interdisciplinary RAI process, ensuring accurate completion, encoding, and electronic transmission of the MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license in good standing.
• Prior experience in a hospital, long-term care facility, or relevant healthcare setting.
• Proficiency in general, rehabilitative, and restorative nursing and medical practices.
• Computer skills including Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience desired.
• BLS/CPR certification required.
