Job Description
The University of Vermont Health Network is seeking a dedicated MDS Coordinator to join our team. Reporting to the Director of Nursing Services, you will collaborate with Nursing Unit Managers to assess resident needs, develop individual care plans, and ensure effective communication within the healthcare team. This remote position requires a strong understanding of the Vermont Nurse Practice Act, Nursing Standards of Practice, and departmental policies.
Responsibilities
- Complete comprehensive resident assessments and create personalized care plans.
- Facilitate communication on resident status and changes with the healthcare team.
- Adhere to professional nursing standards and promote the organization's goals.
Requirements
- Graduation from an accredited Nursing Education Program with an RN or compact licensure.
- Minimum of 5 years of MDS experience post-2019.
- Three to five years of experience in long-term care as a Team Leader or Charge Nurse.
