As part of our commitment to high-quality care and compassion, you will play a vital role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for MDS completion and transmission.
• Contribute to the development of personalized resident Care Plans in alignment with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• LPN license in good standing.
• Prior experience in healthcare facilities.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in Windows applications; AccuMed experience beneficial.
• Strong knowledge of MDS and Care Plan completion.
• Expertise in RAI/PPS processes.
• Current BLS/CPR certification.
