Licensed Practical Nurse MDS Coordinator

Job Description

We are looking for a dedicated Licensed Practical Nurse (LPN) MDS Coordinator to join Genesis Health, a leading healthcare services provider known for excellence and compassion. In this role, you will play a key part in the Resident Assessment Process by ensuring accurate completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to facilitate optimal patient care and reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, ensuring compliance with guidelines and electronic transmission of MDS.
• Contribute to individualized resident Care Plans, aligning with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license and in good standing.
• Prior experience in healthcare facilities.
• Proficiency in general nursing practices, medical procedures, and long-term care regulations.
• Computer literacy including windows applications, AccuMed experience preferred.
