Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
