Job Description

We are looking for a passionate and dedicated Licensed Practical Nurse (LPN) to join Genesis Health's team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs), ensuring optimal patient care and nursing center reimbursement. If you are a licensed LPN with experience in healthcare facilities, proficient in MDS completion, and dedicated to providing high-quality care, we would like to hear from you.

Position Highlights

• Direct the RAI process and ensure accurate completion, encoding, and transmission of the MDS.
• Develop individualized resident Care Plans based on resident needs and regulatory requirements.
• Train interdisciplinary team members on clinical content and electronic MDS completion.
