Genesis Health is looking for a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team as an LPN MDS Coordinator. In this role, you will play a critical part in the Resident Assessment Process, ensuring proper patient care and accurate reimbursement for services provided. Responsibilities

• Direct and oversee the Resident Assessment Instrument (RAI) process in accordance with guidelines.
• Develop individualized resident Care Plans based on resident needs and regulatory requirements.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in long-term care or healthcare facilities.
• Familiarity with general nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer applications, including AccuMed (preferred).
• Previous experience as an MDS Coordinator is a plus.
• Current BLS/CPR certification.
