Job Description
Join us at Heartland Hospice as a full-time Hospice Registered Nurse! As a member of our compassionate team, you will provide direct patient care services and emotional support to patients and their families. This role involves collaborating with the interdisciplinary team to ensure comfort and dignity for patients. Your responsibilities will include managing patient clinical records, participating in quality assessment programs, and maintaining a trusting relationship with patients and families.
Responsibilities
- Act as the primary liaison between physicians, patients/families, and the hospice team.
- Provide direct patient care services and supportive care to patients and families.
- Educate and empower patient caregivers on caring for their loved ones.
- Collaborate with the interdisciplinary team to ensure patient comfort and dignity.
- Evaluate patient and family needs and assist in developing care plans.
Requirements
- RN license in the state of employment.
- 1+ years of nursing experience preferred.
- Compassionate and empathetic approach to patient care.
- Current driver's license and CPR certification.
