Hospice Registered Nurse

Job Description

We are looking for a dedicated Hospice Registered Nurse to join our team at Gentiva Hospice, a leading provider of hospice, palliative, home health, and personal home care services. As a Hospice RN / RN Case Manager, you'll play a critical role in providing compassionate care to patients and their families during challenging times.

Responsibilities

• Coordinate care for patients in collaboration with physicians and hospice team.
• Provide direct patient care services and support to families with empathy and compassion.
• Educate and empower caregivers to ensure the well-being of their loved ones.
• Maintain accurate patient clinical records and participate in quality assessment programs.

Requirements

• RN license in the state of employment (or eligibility to obtain).
• Experience in hospice, home health, ICU, or related fields preferred.
• Demonstrated ability to be compassionate, flexible, and composed in challenging situations.
• Current CPR certification and valid driver's license with automobile insurance.
