Job Description
We are seeking experienced and dedicated Home Care LPNs to join our team at ChristianaCare. As a Home Care LPN, you will provide skilled nursing care and treatment to geriatric patients in the community. You will be responsible for implementing care plans, participating in discharge planning, and ensuring patient-centered care.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement care plans that are goal-oriented and patient-centered.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
