Job Description
We are currently seeking a dedicated Home Care LPN to join our team at ChristianaCare. As a Licensed Practical Nurse (LPN) in our Home Health department, you will play a crucial role in delivering skilled nursing care to geriatric patients in the community. You will work closely with our supportive team to implement care plans, participate in discharge planning, and ensure high-quality patient-centered care.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or equivalent required.
- Current DE LPN license or compact state license.
