Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans and assist in discharge planning.
- Support New Castle County, DE, on day or night shifts, weekends, and holidays.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
