Your responsibilities will include:

• Providing skilled nursing care and treatment to geriatric patients in the community.
• Implementing patient-centered care plans with available resources.
• Participating in discharge planning for patients.

Requirements:

• High School Diploma or GED.
• Current DE LPN license or compact state license.

Job Types:

• Per diem, Part-time, Full-time

Pay:

$22.00 - $28.00 per hour

Expected Hours:

20 – 40 per week
