Care Coordinator AIM

Job Description

We are looking for a Care Coordinator AIM to join our team at Empatia, an industry leader in hospice, palliative, home health, and personal home care services. As a Care Coordinator AIM, you will be responsible for coordinating and managing various operational activities related to the administration of the advanced illness management (AIM) and palliative care department.

Responsibilities

• Coordinate and direct day-to-day operational activities of the AIM and palliative care department.
• Process referrals and ensure timely admission of patients.
• Maintain effective communication with patients, families, and referral sources.
• Coordinate scheduling for Nurse Practitioners and ensure reimbursement processes.
• Manage clinical records functions and maintain compliance with regulations.
• Establish and maintain relationships with referral sources and act as a resource for customers.
• Participate in quality assurance and performance improvement activities.
• Implement marketing and promotional initiatives as directed.

Requirements

• High school graduate or equivalent. RN, LPN, or MA preferred.
• Minimum of three years of healthcare delivery or related business experience.
• Knowledge of medical terminology, computer applications, and insurance reimbursement process.
• Strong data entry, problem-solving, and communication skills.
• Ability to travel as required.
